• By 15 February, ask your employer to make an annual statement of tax paid in advance
• Ask your employer to issue you with a Tax Payment Certificate
• You can use this Confirmation to find out the date of payment and calculate the 2% of your tax paid – this is the maximum amount you can transfer to the recipient. However, this amount must be at least €3.
• Download and fill in the Declaration of the 2% tax remittance.

Information you need to include on the forms:

• Business name or title:
• Seat – municipality, postcode, street, number:
• Legal form:
• Identification number (ID number/SID):

Both of these forms (the Declaration together with the Acknowledgement) must be received by 30. April to the tax office where you live.

• Complete your tax return properly.
• Calculate your 2% personal income tax – this is the maximum amount you can donate to us, but the minimum is 3 Euros.
• In the tax return for individuals, there are already boxes for remitting 2% of the tax to 1 beneficiary (Type A Section VIII of the return, Type B Section XII of the return).

Deliver your duly completed tax return to the tax office of your place of residence by the deadline for filing your tax return (usually 31 March).

• Complete your tax return properly.
• Calculate your 1% corporation tax – this is the maximum amount you can donate to us. You can designate one or more beneficiaries, indicating the amounts involved (but not less than €8 each).
• In the tax return for legal entities, there are already boxes for transferring 1% of the tax to 3 beneficiaries (Part IV of the return).

If you have chosen more than three beneficiaries, include an additional sheet of paper as an attachment to your tax return.

• Deliver your properly completed tax return to your tax office (usually the tax office where you are based) by the deadline for filing your tax return and pay the income tax by that deadline.
